An application for a permit for a temporary closure of a public right-of-way shall be made upon a form provided by the city and obtained in the office of the city manager.  This application shall contain the following information:

    1. The name, residence and business, and phone number of each person and organization sponsoring a temporary closure of a public right-of-way;

    2. The dates and the beginning and ending hours of such temporary closure;

    3. The block in which such closure will occur;

    4. The purpose of the temporary closure;

    5. Such other information as the city manager deems reasonably necessary in order to carry out the provisions of Section 17.04.110 through 17.04.140.  (Ord. 85/T-4153).
