Section 4.64.245 Approval or denial of application for permit.

    1. All permits shall be numbered consecutively and the city shall furnish to each approved applicant a permit setting forth the following information: name, address, date of birth, sex, color of eyes, height, weight, state chauffeur's license number, expiration date of taxi permit, signature of permittee, and further providing space for photograph of permittee.
    2. Upon approval of the application the city shall furnish a permit.  Such permit shall be at all times clearly displayed in the vehicle for hire operated by the permittee and in such a manner as to be readily examined by the police and passengers.
    3. Such permit shall not be transferable and shall be in effect from the date of issuance to the last day of September next succeeding the date of issuance, unless sooner suspended or revoked as in this chapter provided.
    4. A duplicate permit may be issued by the city to replace any license previously issued hereunder which has been lost or destroyed upon the permittee filing a statement of such fact and at the time of filing such statement paying to the city treasurer a duplicate permit fee of $10.00.
    5. Any applicant who is denied a permit by the city clerk may request a hearing before the city manager or his designee by filing a written appeal with the city clerk within ten days of the denial.  The appeal shall be heard by the city manager within 15 days of the date of the notice of the appeal.  The applicant may be represented by counsel and may be allowed to submit a written statement in his/her defense prior to the hearing as well as presenting evidence at the hearing.
    6. Upon final denial of an application for a permit by the City Manager, the applicant shall not be able to reapply for a permit for 12 months from final date of denial unless applicant corrects the reason for denial sooner.   (Ord. 2006-0415; 2001-0340; 98/U-6342; 87/T-6015)
