Section 4.68.040 Applications for ambulance license.

Applications for ambulance licenses under this chapter shall be made upon such forms as may be prepared or prescribed by the licensing authority and shall contain the following information:

    1. The name and address of the applicant and the owner of the ambulance;

    2. The trade or other name, if any, under which the applicant does business and proposes to do business;

    3. The training and experience of the applicant and the transportation and care of the patients;

    4. A description of each ambulance, including the make, model, year of manufacture, motor and chassis number, current state license number; the length of time the ambulance has been in use, and the color scheme, insignia, name, monogram or other distinguishing characteristics to be used to designate applicant's ambulance;

    5. For each ambulance the applicant shall provide an Iowa state service inspection report and a safety inspection report showing that the ambulance is in good condition.

    6. The location and descriptions of the place or places from which it is intended to operate.

    All applications for ambulance licenses to be effective on January 1st of each year shall be on file with the city no later than thirty days immediately preceding the effective date. (Ord. 98/U-6342; 97/U-5245; S-17734, 1971)
