Chapter 4.82 BODY TATTOOING REGULATIONS
Section 4.82.050 Client counseling.
Prior to application of a tattoo on any client the tattoo artist shall:
1. Require the client to fill out and sign a form containing the following information:
a. Name, address, and phone number;
b. The tattoo they are to receive and its location on
the body.
c. Any medical problems that can be vital to safe performance
of a tattoo, or any problems that can
cause the client difficulty afterwards e.g., diabetes, allergic reaction to certain inks, aftercare
products,
medications, hemophilia or blood clotting problems.
d. A list of any medications that may affect the tattooing
procedure such as: anticoagulation
medications;
e. An acknowledgement that the tattoo artist has explained
to them the potential problems and risks
associated with the tattoo they are about to receive and any particular problems they may expect with
the
chosen tattoo location;
f. A copy shall be retained by the tattoo artist for
at least two years. (Ord. 2006-0832)
