Documents required: social security cards for all family members, an official birth certificate for each household member (naturalization papers for non-U.S. birth certificates), and a government issued photo identification (such as a driver's license) for each adult is required within *** 10  DAYS *** of completing the online application.

The required documents can also be submitted in person or mailed (please do not mail originals) to our office. Faxes are not accepted. We do not accept documents by email due to security risks.
