The Division’s Medical Intensive Care Ambulances are strategically located in fire stations throughout the city. These ambulances are supplied with state-of-the-art equipment, and coupled with our highly trained personnel, are capable of handling a variety of life saving procedures. Besides the three full-time staffed ambulances, the division can quickly activate an additional ambulance during times of high call volume.

The EMS Division is supervised by an EMS Director with a line staff of three officers, a force of 24 Emergency Medical Technicians, a Compliance Officer and a Division Training Officer.

Ambulance fees were established based upon past rates charged to 911 customers in Sioux City.   (On Scene Care is only billed if a significant amount of supplies are utilized to stabilize a patient who is refusing transport to the hospital.)

Where will the bill come from?

Billing is contracted with LifeQuest Services of Wautoma, WI, which provides professional and courteous service for our customers.  LifeQuest can be reached at 1-800-786-4911  to discuss any insurance or billing questions.  The City of Sioux City and SCFR Offices are not be equipped to accept ambulance payments at this time.

Insurance information

Ambulance transport is generally covered by most insurance carriers, including Medicare and Medicaid.  Ambulance billing is subject to your insurance policy deductibles and co-pay amounts.  You may be asked to submit insurance information to LifeQuest Services if the correct information was not available at the time of service.   It will also be necessary to get a signature from the patient or family at the time of service, or at the request of LifeQuest Services, in order to process your claim with your insurance carrier.
