The City partners with Wellmark Blue Cross and Blue Shield for our medical plan.   We offer a Preferred Provider Organization (PPO) plan with low out of pocket costs.

 Wellmark offers a variety of tools to help you manage your coverage through your MyWellmark site.  You can

The City of Sioux City partners with Carelon Rx to provide prescription coverage. To view your personal information, you can log on to The Carelon Rx website. On this website you can,

• View your recent prescription activity
• Track the status of any specialty medications
• Price medications as a variety of locations
• View your plan usage
• Find an in-network pharmacy
• Visit the Frequently Asked Questions page to get answers on common questions
• View your prescription cards

Carelon Rx also has an app that allows users access to the same information as well as providing your card on your phone.

Delta Dental is our Dental Plan administrator. Our dental plan covers preventive, basic and major services as well as orthodontia for both children and adults. The per-person annual maximum benefit is $1,200 dollars and is separate from the $2,000 lifetime orthodontia limit.

• Dental Summary Plan Description

VSP provides our Vision coverage for our full-time employees and benefit eligible part-time employees. This coverage allows you to get an eye exam and purchase glasses and/or contacts at a significant savings. It also provides discounts for other services such as Lasik. IBC administers our Flexible Spending Accounts. We offer both Dependent Care and Healthcare Flexible Spending Accounts. These accounts are a way to set aside money, pre-tax, each pay period to pay for eligible dependent care/healthcare expenses.  Each year the City offers the maximum allowed under the IRS at the time Open Enrollment occurs.  Please refer to the Benefit Guide for the annual contribution amounts.

The City has partnered with Madison National to provide all regular full time employees and contract-designated part-time employees a basic life insurance benefit at no cost to them. Eligible employees also have the option to purchase additional, supplemental life insurance in increments of $10,000 up to $300,000*.

• Life Insurance Certificates

Eligible employees also have the option to purchase Spouse Life and Child Life. Spouse Life is available in increments of $5,000 up to $150,000* and Child Life is available in flat amounts of $5,000, $10,000 and $15,000.

Tthe City of Sioux City has partnered with Madison National to provide most regular full-time employees a Long Term Disability benefit provided at no cost to the employee. (Employees in Public Safety are covered under Chapter 411).   In the event of a qualifying illness or injury, employees may be eligible to receive.  This coverage is determined by a number of factors including your ability to perform your own or any occupation.   You are eliglble to participate in the plan following six (6) months of continuous full-time service.  To begin the process for applying for LTD or if you have questions regarding the plan, please reach out to Human Resources.  

• Long Term Disability Certificates

Employees who experience a bona fide retirement will have 20% of their remaining sick leave balance deposited into a Health Reimbursement Account.  These funds are available to pay for retiree health premiums and other eligible expenses.  For more information, please visit the Pension, Deferred Comp, HRA page. 

As outlined by your contract or manual, upon a qualified IPERS/MFPRSI retirement, you will receive 20% of your sick leave balance deposited into an HRA. Please refer to your benefit coverage to see if this is currently available to you.

The City of Sioux City offers a Wellness Program to promote and improve employee health and wellbeing by earning points for completing a variety of activities, tools and resources via the Wellness Portal.  Participating employees who meet the points requirements for both measurement periods can qualify for a Wellness Incentive each fiscal year.
